Dual Laser Treatment Approach



Moles can develop when melanocytes abnormally increase in number in the epidermis or dermis.

Even if it appears to be a simple pigmented lesion, the appropriate treatment method varies depending on its actual location and depth.

Erbium-YAG laser is primarily used for precisely exfoliating shallow surfaces. This laser helps effectively ablate shallow moles in the epidermis while minimizing irritation to the surrounding skin.

It can be particularly useful for facial mole removal, as it reduces the risk of scarring after treatment and allows for aesthetic improvement. Subsequently, finishing the base of the mole with CO2 laser using thermal energy can help destroy any melanocytes that might remain deep, thereby reducing the possibility of the mole reappearing. This is beneficial in preventing recurrence, which can happen if the root remains, even if the mole appears removed on the surface.

A CO2 laser finish can be considered in the following cases:

-If the mole is slightly raised visually or feels palpable on the skin when touched

-If a mole has recurred in the same location before

-If diagnosed as a dermal nevus, or appears to be a deep-seated lesion such as a blue mole or a grayish-brown mole


*Flat warts are viral skin diseases caused by human papillomavirus (HPV) infection. They typically spread widely in the epidermal layer and appear as small, thin, flat, brownish or flesh-colored lesions to the eye. They are prone to appearing when immunity is weakened and can spread to other areas through self-contact, so early management is recommended.

Erbium-YAG laser can be used to uniformly ablate the epidermal layer, making it effective for widely spread flat wart lesions.

It helps to remove them efficiently. It allows for even treatment of the lesion area while minimizing skin irritation. However, viral lesions carry a risk of recurrence if only the surface is treated. In such cases, a finishing step using a CO2 laser to deliver subtle thermal stimulation around the lesion can be a clinically significant approach to further eliminate virus-infected cells that may remain in the epidermis and surrounding areas, thereby reducing the possibility of recurrence.

A finishing process using CO2 laser can be particularly helpful in the following situations:

*Milia are lesions that appear as small white sacs due to the accumulation of keratin (a type of skin protein) beneath the skin. They primarily occur around the eyes, on the cheeks, and forehead, and unlike sebaceous cysts, they are considered a cosmetic concern unrelated to infection or inflammation.

Erbium-YAG laser can be effectively used to remove milia. It precisely exfoliates the skin surface to refine the thin surface of the milia and creates a pathway for the trapped keratin to be effectively discharged. Lightly finishing with a CO2 laser after treatment can reduce the possibility of remaining keratinocytes proliferating again and help manage the recurrent appearance of milia in the same area. However, excessive thermal stimulation to the skin can cause hyperpigmentation, so careful control of intensity is required.

Erbium-YAG laser can precisely and delicately remove the epidermal layer, making it suitable for treating syringomas in sensitive areas like around the eyes.
